Occasionally lifting, pushing, and/or pulling objects weighing up to or over 50 lbs.; intermittently sitting at a computer workstation for extended periods of time; and some occasional job-related travel.

Monday through Friday, 8:00 a.m. – 5:00 p.m.

Job Summary

This position will work closely with both inpatient and outpatient providers of the Advanced Heart Failure service to improve care coordination for the heart failure population.

Skills / Knowledge / Abilities

The ideal candidate for this position will be a self-starter, an independent thinker, as well as have attention to detail.

Cardiovascular experience preferred. Ambulatory experience preferred.
